Memorial Hospital, in partnership with Community Health Collaborative, seeks to engage the greater community using the Community Health Needs Assessment (CHNA) report. Local and regional planning is a critical part of the needs assessment and health improvement process.
Once the Carroll County community has reviewed the CHNA report and data presentation, the input will be utilized to generate Memorial Hospital’s health improvement strategy.
Through the community engagement process, Memorial Hospital and our community partners aim to:
- Ensure interests of the local community are represented
- Obtain stakeholder input on identifying significant health needs based on review of shared CHNA data
- Solicit stakeholder feedback to prioritize significant health needs
- Identify local assets and resources that may address local health priorities:
- Who or what organizations are addressing the need?
- What infrastructure (assets) is available to assist community members in addressing needs? (e.g. public transportation, bi-lingual community leaders).W
- What are gaps related to the need and what resources would address gaps?
Memorial Hospital invites broad representation at the Community Engagement forum(s) such as:
- Public health and Community health coalitions
- Healthcare providers, including oral and behavioral health care providers
- Minority populations (e.g. NAACP members, GSTA, LGBT, ethnic groups)
- Business and civic leadership (e.g. local employers, civic organizations, community leaders)
- Funding agencies (e.g. local philanthropic organizations, bank & credit unions)
- Local and state government
- Non-profit organizations, including other healthcare providers, and social service agencies
- Childcare providers, Schools (Pre-school through High School), and Colleges or Universities
- Veterans

Memorial Hospital will post meeting notes on their website, making community input available for any interested party to review and use.
